NRC ROUTINE RADIATIONPROTECTION INSPECTION REPORT NO.
50-331/96009(DRS)
Dear Mr. Liu:
This refers to the inspection conducted by K. N. Selburg of this office on October 21-25,
1996.
The inspection included a review of authorized activities for your Duane Arnold
Energy Center, Unit 1 facility.
The inspection was an examination of activities conducted under your license as they
relate to radiation safety and to compliance with the Commission's rules and regulations
and with the conditions of your license.
The inspection consisted of a selective
examination of procedures and representative
records, observations,
and interviews with
personnel.
Several good initiatives were noted in the radiation protection program, including the use
of ALARApersonnel as drywell and refueling floor coordinators, and the use of electronic
dosimetry and audiovisual equipment during the refueling outage.
Depleted zinc injection
and the chemical decontamination
performed during the previous refueling outage
contributed to a significant reduction in overall general area radiation levels.
These
initiatives contributed to a low outage dose.
While good initiatives continued to be implemented, communication problems between
radiation protection personnel and other departmental
personnel were observed.
These
cOmmunication problems contributed to the following licensee-identified events:
maintenance
unawareness
of a requirement in a radiation protection procedure to leak test
certain station hoses; a maintenance worker inadvertently approaching
a high radiation
area; workers breaching a contaminated valve without health physics coverage; and a
contamination spread during the performance of a surveillance test.
The weaknesses
in
communication were present with all involved departments.
These communication
problems were noted through the end of the inspection period.
Based on the results of this inspection, certain of your activities appeared to be in violation
of NRC requirements,
as specified in the enclosed Notice of Violation (Notice). The
violation concerns the failure to comply with an approved administrative control procedure.

This violation is of concern because
your procedure requiring leak testing hoses was not
implemented for any of your breathing air and plant service hoses.
Please note that you
are required to respond to this letter and should follow the instructions specified in the
enclosed Notice when preparing your response.
The NRC will use your response,
in part,
to determine whether further enforcement action is necessary to ensure compliance with
regulatory requirements.
In accordance with 10 CFR 2.790 of the Commissions regulations, a copy of this letter
and the enclosures will be placed in the NRC Public Document Room.
