REPORT
NO. 50-331/96006(DRP)
Dear Mr. Liu:
On September
6,
1996, the
NRC completed
a routine safety inspection at your
Duane Arnold Energy Center
(DAEC) facility.
The enclosed
report presents
the
results of that inspection.
During this 7-week inspection period, your conduct of activities at the
Duane
Arnold Energy Center facility was characterized
by
a safety conscious
approach
to activities.
We are concerned,
however,
about
an emergent
negative trend in
human performance
events.
This report discusses
several
examples
where
human
error contributed to poor exe'cution of safety
system work.
Additionally, this
report also identifies several
areas of weak oversight
and control of
contractor activities.
While the safety consequences
of these
examples
were
minor,
we are concerned
about these
items in light of the large
number of
contractors
anticipated
on site to support your upcoming refuel outage.
Additionally, the
NRC has determined that
2 violations of NRC requirements
occurred.
The violations are cited in the enclosed
the circumstances
surrounding
them are described
in detail in the subject
inspection report.
The first violation is of concern
because
of the
importance of assuring
proper valve line-up configurations
on safety-related
equipment.
The second violation is of concern
because it represents
an
example of weak oversight of contractor activities.
The
NRC has concluded that information regarding the reason for the
violations, the corrective actions
taken
and planned to correct the violations
and prevent recurrence,
and the date
when full compliance
was achieved is
addressed
in the subject inspection report.
Therefore,
you are not required
to respond to this letter unless
the description therein
does not accurately
reflect your corrective actions or your position.
In that case,
or if you
choose to provide additional information, you should follow the instructions
specified in the enclosed
Notice.
