INTEGRATED INSPECTION
REPORT
NO. 50-331/96-05
AND
Dear Hr. Liu:
On July 18,
1996,
the
NRC completed
a routine safety inspection at your Duane
Arnold Energy Center
(DAEC) facility.
The enclosed
report presents
the
results of that inspection.
During this 6-week inspection period,
you exhibited
a safety conscious
approach
toward activities at the
Duane Arnold Energy Center.
We are
concerned,
however,
about
2 violations of NRC requirements
that were
identified.
NRC inspectors
identified that the safety evaluation to support
the nonconforming
emergency
makeup flow rate to the spent fuel
pool
was inadequate.
That safety evaluation did not provide sufficient bases
to demonstrate
that the condition did not involve an unreviewed safety
question.
NRC inspectors
also identified several
procedure deficiencies
during the course of an operator license examination.
We are concerned
that
these deficiencies
are similar in nature to those identified during the

prior operator licensing examination
inspections.
These violations are cited in the enclosed
Notice of Violation, and the
circumstances
surrounding
the violations are described
in detail in the
enclosed report.
Please
note that you are required to respond to this letter
and should follow the instructions specified in the enclosed
Notice when
preparing your response,
The
NRC will use your response;
in part, to
determine whether further enforcement
action is necessary
to ensure
compliance
with regulatory requirements.
This report discusses
several
examples of poor interdepartmental
communications.
These
problems
occurred during the performance of routine
activities
and involved multiple departments.
Although there were
no
significant consequences
as
a result of the examples listed,
we are concerned
about this'issue
due to the importance of good communications
in ensuring
proper conduct of routine evolutions.
