Inadequate trend evaluation of 10 amp and 15 amp fuse inspections failures results in preventable fuse failures.

There were no major team inspections in 2016 Failure to address degraded turbine roof vents results in generator control cabinets water intrusion and a reactor trip Failure to provide appropriate work instructions for instrument seal assembly results in failure of some instrumentation designed for post-accident monitoring 4
