This refers

to the

inspection

conducted

by

S.

A. Elrod of this office on

November

5 to

December

23,

1991.

The

inspection

included

a

review of

activities authorized for your St.

Lucie facility.

At the conclusion of the

inspection,

the findings

were

discussed

with those

members

of your staff

identified in the enclosed

report.

Areas

examined

during the inspection

are identified in the report.

Within

these

areas,

the inspection consisted

of selective

examinations

of procedures

and

representative

records,

interviews with personnel,

and

observation

of

activities in progress.

Based

on the results

of this inspection,

certain of your activities

appeared

to

be in violation of

NRC requirements,

as specified in the enclosed

Notice of

Violation (Notice).

We are

concerned

about violation "A" because

of the

failure to properly 'proceduralize

Technical

Specification

required

chemical

analysis for emergency

diesel-generator

(EDG) fuel oil.

This resulted

in a

common. mode failure, in that it caused

three of the 'four onsite

EDGs to be

inoperable.

Your corrective action

was prudent

and demonstrated

the 'meticulous

care

needed

in this area.

We are also concerned

with your control of outage

maintenance activities, which resulted

in two violations.

You are required to respond

to this letter

and should follow the instructions

specified

in the

enclosed

Notice

when

preparing

your response.

In your

response,

you should

document

the specific actions

taken

and

any additional

actions

you plan to prevent recurrence.

After reviewing your response

to this

Notice, including your proposed

corrective actions

and the results

of future

inspections,

the

NRC will determine

whether further

NRC enforcement

action is

necessary

to ensure

compliance with NRC regulatory requirements.

In accordance

with 10 CFR 2.790 of the

NRC's

"Rules of Practice,"

a copy of

this letter and its enclosures will be placed in the

NRC Public Document

SUMMARY

Scope:

This routine resident

inspection

was

conducted

onsite

in the

areas

of plant

operations

review,

maintenance

and plant, outage

observations,

surveillance

observations,

refueling activities,

review of special

reports,

and review of

nonroutine events.

Results:

During this inspection

period

many operational activities were

performed in a

superior

manner,

but

a Unit

1 refueling

outage

was

complicated

by several

events

involving support activities.

The major events

were:

bending of two

Unit

1 reactor

core instrument stalks; particulate

contamination of emergency

diesel

generator

(EDG) fuel oil storage

tanks;

chloride contamination of the

Unit

1 reactor coolant system

(RCS);

and

a bent control element

assembly.

None

of the events

overly delayed

the outage,

or required Technical Specification

(TS) relief.

The licensee

did respond

to events

in a responsible

fashion.

Within the

areas

inspected,

the following violations

(VIOs)

and unresolved

items

(URIs) were identified:
