ATTACHMENT FINDING:

During the Nuclear Regulatory Commission (NRC) inspection conducted on November 1 December 5, 1987, a violation of NRC requirements was identified. The violation involved entering an operational mode without the necessary limiting conditions for operation (LCO) being met. In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1987), the violation is listed below:

Unit 1 Technical Specification (TS) 3.0.4 requires that entry into an operational mode shall not be made unless the limiting conditions for operation (LCO) are met without reliance on provisions contained in the action statement, unless otherwise expected.

Contrary to the above, on October 14, 1987, Unit 1 entered Mode 4 from Mode 5 without the conditions of LCO 3.6.6.2 being met. Shield building integrity was not being maintained due to a door that was left open and unattended.

RESPONSE

1. Florida Power & Light Company concurs with the violation.
2. Procedural deficiency and personnel error in paperwork review by the operators involved were the cause of the finding.
3. Shield Building integrity was immediately established.

The shift was conuseled on the need for an accurate and thorough review of shift paperwork prior to mode changes. Additionally, shift meetings were held by the Operations Supervisor on November 30, 1987, December 7, 1987, and January 19, 1988, to discuss the details of this report. The need for thoroughness and accuracy in review of paperwork and operator responsibilities was also emphasized.

4 ~ OP1-0030120, Pre-start Check-Off, will be revised, adding a sign-off signifying that vital areas are revitalized prior to entry into Mode 4. This will ensure that the Shield Building door and all similar areas are locked or controlled prior to required mode changes.

5. Implementation of these measures will be completed by March 31, 1988.
