NOTICE OF VIOLATION

(NRC INSPECTION

REPORT

NO. 50-335/86-01)

This refers to the Nuclear Regulatory

Commission

(NRC) inspection

conducted

on

January

8-9 and February 25-26,

1986, at your St.

Lucie facility.

As

a result of

this inspection, signific.nt failures to comply with NRC regulatory requirements

were identified,

and accordingly,

an Enforcement

Conference

to discuss

these

matters

was held in Miami, Florida on February

25,

1986.

The violations described

in the enclosed

Notice of Violation include the failure

to establish

radiological control. procedures

for steam generator

work and to

perform adequate

surveys (evaluations)

of individual exposures

during the

steam generator

work.

The

NRC is concerned that these violations, collectively,

caused

a significant potential for personnel

exposures

in excess

of NRC limits.

You stated during the Enforcement

Conference

of February

25,

1986, that the root

cause of this event

was the underresponse

of your, direct reading dosimeters

when

they were exposed parallel to the radiation field.

In our view,

a more basic

cause of this event

was failure to establish

procedures

and to perform adequate

surveys prior to the sludge lancing work.

In accordance

with the "General

Statement

of Policy and Procedure

for NRC

Enforcement Actions,"

10 CFR Part 2, Appendix

C ( 1985), the violations have

collectively been categorized

as

a Severity Level III problem.

Normally,

a

civi 1 penalty is considered for a Severity Level III violation.

After

consultation with the Director, Office of Inspection

and Enforcement,

I have

decided that

a civil penalty will not be proposed

in this case

because

of your

good prior performance

in the area of concern

as indicated

by the absence

of

significant violations

and by Category

1 ratings in the Radiological

Controls

area during your three

most recent

Systematic

Assessment

of Licensee

Performance

(SALP) periods.

You are required to respond to this letter and should follow the instructions

specified in the enclosed

Notice when preparing your response.

In your response,

you should document

the specific actions

taken

and any additional actions you

plan to prevent recurrence.

After reviewing your response

to this Notice,

including your proposed corrective actions,

the

NRC will determine

whether
