LUCIE UNIT 1 DOCKET NO. 50-335 IE INSPECTION REPORT 81-31 FINDINGS 10 CFR 50, Appendix B
Criterion V requires that activities affecting quality shall be pr escr ibed by documented instructions, procedures or drawings and accomplished in accordance with these instructions procedures and drawings.
1.
Florida Power and Light Company guality Instruction 10.23, Revision I, paragraph 4.3.5, requires that expansion anchors shall be seated to at least the minimum embedment depth as noted in the manufacturers instructions, or applicable design document; EBASCO specification FLO 9110-AS-13 Table 4.03( 1) requires a
minimum embedment depth of 6.5 inches for 3/4 inch diameter expansion anchor bolt.
Contrary to the above.
the 3/4 inch diameter expansion anchor bolts for pipe restraint CH-3-10A were installed, inspected and gC accepted with an embedment depth of appoximately 2 1/2 inches.
2.
Florida Power and Light Company equality Instruction
- 10. 14, paragraph
- 5. 2.3. 4, requires gC verification that the pipe hanger assembly is i n accordance with approved drawings.
Contr ary to the
- above, gC inspected and accepted pipe hanger CCH-169 was not in accordance with approved detail drawing in that it had a
fastener arrangement that differed from the arrangement required by the detailed drawing.
RESPONSE
1.
FPL concurs with the findings.
2.
These findings were caused by personnel error.
3.
As corrective
- action, non-conformance reports were issued for these
- items, and they were properly dispositioned.
4.
As corrective action to prevent future
- problems, quality control inspectors have been reinstructed in the requir ements for hanger inspections.
In addition a
review was conducted of anchor bolts in backfit stores for proper length.
Anchor bolts 3/4" by 5 1/2 have been determined unsuitable for use at this time.
A review of plant changes using the 5
1/2" anchor bolts was conducted.
This review identified potential problems in two additional hangers.
These potential problems have been properly dispositioned.
5.
Full compliance was achieved January 7, 1982.
