APPENDIX A NOTICE OF VIOLATION Florida Power and Light Company St. Lucie License No.
CPPR-144 Based on the NRC inspection September 17-19, 1980, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below.
These items have been categorized as described in correspondence to you dated December 31, 1974.
A.
As required by Paragraph (a)(1) of 10 CFR 50.55a,
"...components...
shall be...
fabricated...
and inspected to quality standards commensurate with the importance of the safety function to be performed".
For component storage and protection this requirement is implemented by Florida Power and Light Company Topical Report (FPSrLTgARl-76A) Appendix C, which requires storage and preservation of components to be accomplished in accordance with ANSI N45.2.2, "Packaging, Shipping, Receiving, Storage and Handling of Items for Nuclear Power Plants (During the Construction Phase)."
N45.2.2, Paragraph 6.1.2(3) states "Ievel C items shall be stored in doors...".
Contrary to the above, on September 17, 1980, six safety related, Level C, ASME Class 1, 2 and 3 valves were stored outdoors.
This is an infraction.
B.
As required by Criterion V of Appendix B to 10 CFR 50 as implemented by the Florida Power and Light Company Topical Report (FPLTgAR1-76A), Section 5, "Activities affecting quality shall be prescribed by documented...
procedures...
and shall be accomplished in accordance with those...
procedures...."
Florida Power and Light Procedure SgP-8, Revision 1, TC g2, "Storage and Distribution of Welding Filler Materials",
Paragraph 8.3.7.1 requires all unused welding filler materials to be returned to the Weld Rod Control Room.
Contrary to the
- above, to the above on September 17,
- 1980, the following examples were noted:
1.
FPSL had no procedural requirement to control used welding filler material.
The above lack of control resulted in numerous past welding areas containing large quantities of abandonded partially used welding fillermaterial.
2.
Thirty-one coated unused welding electrodes were abandoned in two locations of the Unit 2 auxiliary building.
This is an infraction.
A similar item was brought to your attention in our letter dated October 12, 1979.
