Certification of gC Inspection Personnel
- Failure to follow Procedure.
I As required by Criterion V of Appendix B to 10 CFR 50 as implemented by the Florida Power 5 Light Company Topical Report (FPLTRAR1-76A), Section 5, activities affecting quality shall be accomplished in accor'dance with established procedures.
Procedure No. (P-2.9, Revision I requires magnetic particle (MT) inspectors to be certified prior to per'forming MT inspections without total supervision.
Contrary to the above, a certified MT inspector allowed one individual who was not certified for MT inspection to perform MT inspections without total supervision during the time frame of August through September 1979.
RESPONSE
Following notification of this apparent problem at: the exit interview, Florida Power 8 Light conducted extensive investigations into the validity, extent, and causes of the problem.
Our investigations identified three instances occurring on September 7th (2), and 14th,
- 1979, where a qualified but uncertified inspector conducted MT inspections, Since no other instances were found during a complete review of all magnetic particle inspection reports for the time frame in question and from interviews with gC personnel, we conclude that these-three occurrances were isolated to one individual on the dates specified.
The problem resulted because field inspection personnel were under the misconception that the individual was certified because he had passed his qualification tests on August 29,
- 1979, when in fact, the individual's Document of gualification had not been signed.
To preclude recurrence of this problem, gC management has re-emphasized it's policy that an individual shall not conduct any inspections until his Document of gualification is on site.
To eliminate possible confusion on the part of field personnel, the back of each individual's ID badge will be stamped to indicate the certifications he holds.
Implementation of these actions is complete.
