APPENDIX A NOTICE OF VIOLATION Florida Power and Light Company St. Lucie Facility License No. DPR-67 Based on the NRC inspection on August 20-24,
- 1979, certain of your activities were apparently not conducted in full compliance with NRC requirements as indi-cated below.
This item has been categorized as described in correspondence to you dated December 31, 1974.
As required by Technical Specification 6.8.1, procedures shall be established and implemented.
Procedure gl 12-PR/PSL-4, Revision 3 dated February 16, 1978, Appendix A requires that when Measuring and Test Equipment (MME) is used, the logbook must be filled out to indicate where the equipment is used.
This procedure further requires that the MME used on a safety-related job shall be noted on the appropriate Plant Work Order (PWO).
Contrary to the above, procedure gl 12-PR/PSL-4 was not implemented in that the safety-related torquing of the closure studs on the Reactor Building Containment Equipment Hatch accomplished on May 29, 1979, according to PWO 3997 was co'mpleted without logging the required information on the calibrated MME torque wrench used on either the PWO or in the MME logbook.
