APPENDIX A NOTICE OF VIOLATION Florida Power and Light Company St: Lucie 2 License No.
CPPR-144 Based on the results of the NRC inspection on June 11-15, 1979, it appears that certain of your activities were not conducted in full compliance with NRC requirements as indicated below.
These items have been categorized as described in our correspondence to you dated December 31, 1974.
As required by Criterion U of Appendix B to 10 CFR 50, as implemented by commitments set forth in the Florida Power and Light (FPSL)
Company QA Topical Report, Section 5.0, paragraph 5.1, "Activities affecting quality of nuclear safety related structure,
- systems, and components shall be prescribed by documented instructions, procedures, or drawings...and shall be accomplished in accordance with these instructions, procedures, or drawings."
FPSL Administrative Site Procedure (ASP) 4 'and Site Quality Procedure (SQP) 7 which superceded the former procedure in June 1979 re-quires when a
change to a controlled design document is approved via a
Field Change Request (FCR)/Design Change Notice (DCN), the reproducible and each controlled copy of an affected document is stamped "This drawing revised by FCR/DCN and the specific FCR/DCN number is printed in the blank.
Contrary to the
- above, FCR No. 2-557 had been incorrectly recorded on the project QC Supervisor's office copy and the QA project office copy of drawing number 2998-G-517R1.
A review of the FCR file disclosed that FCR No. 2-434 was applicable, but was not recorded on the subject drawing.
This is an infraction.
This is a continuing item of noncompliance first identified on February 21,
- 1979, which was not fully corrected in accor-dance with your response letter dated March 12, 1979.
