MVlO FOR FACSIMILE TRANSMISSION TO CONFIRM PRO/PT REPORTABLE OCCURRENCE 335-78-42 This memo is to confirm the telephone notification of the subject report-able occurrence made Saturday, November ll, 1978 to Mr. E. Webster of your office.

During full power operation following a maintenance

outage, a licensed operator discovered 4

NAOH Addition System Valves de-energized while performing routine observation of the reactor/turbine generator control board in the control room.

Following investigation, it was found that the fuses for these valves were removed.

This disabled the system which is contrary to Specification 3.6.2.2.

The fuses for these valves (SE 07-1A, -1B, -2A and -2B) were removed to de-energize the valves during the reactor plant cooldown prior to the mainte-nance outage as required by Operating Procedure 0030127.

During the reactor plant heatup following the maintenance

outage, the fuses for these valves were not replaced.

Replacement of the fuses to energize the valves is required by Operating Procedure

0030121, but was inadvertently missed during startup.

The fuses were immediately replaced and the valves were verified energized.

The valves had been de-energized for approximately 57 hours6.597222e-4 days <br />0.0158 hours <br />9.424603e-5 weeks <br />2.16885e-5 months <br />.
