NOTICE OF VIOLATION Convnonwealth Edison Company Dresden Station, Units 2 and 3 Docket Nos.

50-237; 50-249 License Nos. DPR-19; DPR-25 As a result of the inspection conducted on July 7 through 31, 1992, and in accordance with the *General Statement of Policy and Procedure for NRC Enforcement Actions,* 10 CFR Part 2, Appendix C~ (1992) (Enforcement Policy) the following violation was identified~

10 CFRPart 50, Appendix 8, Criterion V, *instructions, Procedures, and Drawings,* requires, in part, that activities affecting quality shall be prescribed by documented instructions, procedures, or drawings of a type appropriate to the circumstances, and shall be accomplished in accordance with these instructions, procedu~es, or drawing~.

Dresden-Administrative Procedure 7-4, *control of Temporary System Alterations,* requires that temporary alterations be positively B *.

identified.*

Contrary to the above, between April 4, 1992, through July 24, 1992, a temporary alteration of a Unit 3 feedwater controller electrical circuit was installed but not positively identified in that the electrical leads were not tagged.

Dresden A~ministrative.Procedure 9-9, *special Procedures (SP),"

requires SPs which have the potential of changing the status of equipment in service, to be entered in the SP In Use Record each time the SP is initiated, suspended, or completed.

Contrary to the.above, as of July 23, 1992, twenty SP suspensions or completions, having the potential of changing the status of equipment

_in-service, were not recorded in the SP In Use Record.

DFPS 4123-05, *2;3 Diesel Fire Pump Weekly Operability Test,* Step 7 of requires that the material condition of the area be checked and cleaned as needed, and step 6 of Attachment 1 ~equires that the battery racks be checked for signs of damage or corrosion.

Contrary to the above, on July 23, 1992, the batteries were dirty and

.covered with acid, and the battery rack was corroded. These conditions existed for a period greater than one week~

Dresden Administrative Procedure 12-25, *Radiation Work Permit Program,"

requires all individuals to sign the RWP-acknowledge-form each day while working on the RWP signifying that the RWP was read, under~tood, and would be complied with.

RWP 2G003A, *Reactor, Turbine, R/W, HRSS,

  • Routine Sa~ple Prep Monitoring Transportation Lab Work Cleaning,"

~equires protective clothing and equipment in contaminated areas.

Contrary to the above, on July 13, 1992, two individuals entered.a Unit 2 reactor building contaminated area without the full protective clothing and equipment required on the RWP.

Dresden Administrative.Procedure 7-5, "Operating logs and Records,"

requires, in part, that when a limiting condition for operation (lCO) entry condition occurs, the event wi 11 be 1 ogged in the lCO 1 og.

  • Contrary to the above, on April 8, 1992, and on June 30, 1992, when the Unit 3 Service Water Effluent Radiation Monitor was declared inoperable~

a lCO entry condition, the event was not logged in the lCO log.

This is a Severity level IV violation (Supplement~).

The inspection sho~ed that steps had been taken to correct the identified violation and to prevent recurrence. Consequently, no reply to the violation

  • is required. and we have no further questions regarding this matter.
