NOTICE OF VIOLATION Cummonwealth Edison Compariy Dresden Nuclear Power Station Units 2 and 3 Docket Nos. 50-237.; 50-249 Licenses. No. DPR-19; No. DPR-25 As a result *of the inspt:ction conducted on Ma.y 21 through June 21, 1990, and in accordance with the "General Statcrr:ent*of Policy and Procedure for NRC Enforcement Actions," 10 *CFR Part 2, Appcridix C, (1990) (Enforcernent Policy) the following violaticins were identified:

10 CFR S0.49, Paragraph (f): r_equfres that each item of electrical equipment important to safety be qualified by testing and/or analysis of an'identical item of equipment under identic~l conditions,* or a similar ite~ or under similar conditions with a. supporting analysis to show that the equipment to b~ qu~lified is acceptable.

Contrary to the above, the. following examples of electrical equipment impot~tant to safety were not propc:rly c;ualified by EQ tests and/or analysis:.

Six splices made of cloth tape.wrapped over plastic in_sulation tape were found inst~lled in th~ powe~ and coritrol ~i~cuits fot the HPCI U~i*t 2 Turbine St~im Supply'I~olat~on Valve.

Two stub splices with wire nuts were-installed on ~he Reactor *Coolant.

~ost~Accident Sample Valve in Unit 3.

Two Voltrex heat shrink splices were install~d in the *heat trace

  • circuit for the Hydrogen Analyzer in Unit 2.

Four Amphenol corinectors encapsulated in Voltrex heat shrink tubing were installed in the instrur;;t:r:t. circuits to the* Unit 3 containment High Range Radiation Monitors.

This is a Severity Level IV violation (Supplement I.D).

Contrary to the above, as of A;:Jrii 1989,--the licensee's EQ files did not contain EQ tests and/or analysis which demonstrated qualification.6f.

GE 2960 terminal blocks, one:,:) Cir.ch.terminal block, one (i) Marathon 6000 terminal block, and eleve~ *~1; elettrical enclosures jnstalled without the requ.ired weepholes in ~i~h Energy Line Break (HELB)/radiation areas. In addition, the licensee's C file for Marathon terminal blocks failed to identify 64 Marathon 1500 !600 EQ terminal blocks which were found installed in EQ circuits in H LB and/or radiation environments.

(1) thE: corrective steps that have been taken and the results achieved; (2) the corrective steps that will be taken to avoid further violations~ and (3) the date when full complia~ce will be achieved.
