Appendix NOTICE OF VIOLATION Commonwealth Edison Company Docket No. 50-237 Docket No. 50-249 As a result of the inspection conducted on February 13-26, 1985, and in accordance with the General Policy and Procedures for NRC Enforcement Actions, (10 CFR Part 2, Appendix C), the following violations were identified:

10 CFR 50.55a(g) requires that licensees implement an inservice testing program for pumps and valves per the requirements of Section XI of the ASME Code unless specific relief has been granted by the Commission.
Subsections IWV-3300 and Table IWP-3100-1 of Section XI of the ASME Code states that remote position indicators for all valves be verified once every two years to accurately reflect the status of the valve and that pump suction pressure be measured with the pump idle.
Contrary to the above, the licensee was not verifying the accuracy of remote position indicators for accessible valves nor measuring pump suction pressures with the pumps idle.
No request for relief from these requirements had been submitted to the Commission.
This is a Severity Level V violation (Supplement 1).

Criterion XII of Appendix B to 10 CFR 50, as implemented by Commonwealth Edison Company Corporate Quality Assurance Manual, Chapter 12, requires that measuring and test equipment be properly calibrated and controlled to maintain accuracy.
In addition, 10 CFR 50.55a(g) requires that licensees demonstrate the operability of pumps and valves by the success-ful performance of inservice tests that adhere to the requirements delineated in Section XI of the ASME Code.
. Contrary to the above, inservice test procedures did not require the use of traceable and calibrated portable measuring and test equipment.
This is a Severity Level V violation (Supplement 1).

Criterion XVI of 10 CFR 50, Appendix B, as implemented by Commonwealth Edison Corporate Quality Assurance Manual, Chapter 16, states that methods shall be established to insure the timely identification of conditions adverse to quality.
In addition,Section XI of the ASME Code, Subsection IWV-3413, requires that valves with stroke time increases in excess of stated percentages be tested at an increased frequency until corrective action has been taken.

Contrary to the above, the licensee failed to evaluate the valve stroke time data for all of 1984 on Unit 3.
This is a Severity Level IV violation (Supplement 1).
With respect to item 2, the inspection showed that action had been taken to correct the identified item of noncompliance and to prevent recurrent.
Con-sequently, no reply to this item of noncompliance is required and we have no further questions regarding this matter.
With respect to items 1 and 3, pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each item of noncompliance:
(1) cor-rective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full compliance will be achieved.
Consideration may be given to extending your response time for good cause shown.
