COMMONWEALTH EDISON COMPANY (DRESDEN UNIT 2)

Problem:

CRACKS IN CORE.SPRAY LINES The licensee informed Region III on January 28, 1975, that cracks were discovered _in both of the reactor c:ore spray lines.

Moisture was.*

observed on each of the core spr~::iy line surfaces when the insulation was re;:noved in preparation for in-service inspection.

The following preliminary information was provided by the licensee:

1. The core spray lines are 10" in diameter, with stainless steel safe-ends (about 14" in length) welded to the reactor vessel nozzles (see attached sketchs).

Short pieces of welded stainless steel pipe called "dutchman" (about 9" in length) connect the core spray *lines with the safe-ends.

a. Two longitudinal cracks were observed at the heat affected zone

.at Point A on the loop "A" dutchman to safe-end weld.

One crack uas 3/4" in length at the 3 o'clock position.

The other crack was 1/2" in length at the 8 o'clock position~

position at the heat affected zone of the dutchman to inlet pipe at Point B on loop "A".

c. Two circumferential cracks were observed at the heat affected zone at Point C on the loop "B" dutchman to pipe weld.

The two cracks, each 1/8" in length, were at the 10 o'clock and 11 o'clock positions.

d. The licensee stated.that all five cracks were observed to be weeping, indicating through wall penetration.
2. Engineering drawings indicate that the safe-ends are made of type 316 furnace sensitized stainless steel.

The dutchman is also made of type 316 ~tainless steel.

The inlet piping is made of type 304* stainless steel.

3. The licensee is preparing to volumetrically examine all other welds in the core spray lines.
4. The reactor is currently shut down for refueling, equipment maintenance and replacement of the 4 recirculation line bypass piping.
5. The licensee is evaluating the pipe failures.

A report will be submitted in accordance.with regulatory requirements.

Action Taken by Inspection and Enforcement:

1. An inspector h2s been dispatched to the site.
2. A press release has been prepared by Region III.
3. Representatives from the State of Illinois will be notified.
