799 ROOSEVELT ROAD GLEN ELLYN, ILLINOIS 60137 December 23, 1980 This refers to the telephone discussions between Mr. Scott and other members of your organization at the Dresden Nuclear Station and Messrs. Reimann and Tongue of this office on December 23, 1980, regarding the inadvertent repressurization of the Unit 3 reactor from cold shutdown conditions on December 20-21, 1980.
With regard to the matters discussed, we under~tand that you will take the following actions to prevent re-
- currence of this event:
- 1.
Complete an onsite review by the onsite review group to determine the safety significance of this event and determine actions necessary to prevent future recurrence prior to restart of Unit 3.
- 2.
Implement a station operating order to assure that* one Shutdown Cooling Ptmlp will.be operated at rated flow during cold shutdown conditions for Unit 2 or 3 to assure adequate mixing of water in the reactor pressure vessel.
A second Shutdown Cooling Ptmlp will be operated as necessary to control coolant temperature.
The opera-ting order will be in effect prior to Unit 3 restart and will remain in effect until permanent procedural and/or design changes are im-plemented to prevent recurrence.
This requirement shall apply only when in cold shutdown condition with no recirculation pumps operating.
- 3.
Provide in the report of this event required by the technical specifications your analysis of the cause of this event and a description of your planned permanent corrective actions.
The report will discuss such items as pr_ocedure changes and design changes to prevent recurrence of similar events, and your schedule for implementing corrective actions.
