NOTICE OF VIOLATION (NRC INSPECTION REPORT NOS. 237 /94005;
249/94005)
Dear Mr. Lyster:
This refers to the inspection conducted by M. Leach, A. M. Stone, C. Phillips,
and D. Chyu of this office on February 23 through April 11, 1994.
The
inspection included a review of activities authorized for your Dresden Nuclear
Station, Units 2 and 3.
At the conclusion of the inspection, the findings
were dis~ussed with those members of your staff identified in the enclosed
report.
Areas examined during the inspection are identified in the report. Within
these areas, the inspection consisted of selective examinations of procedures
and representative records, interviews with personnel, and observation of
activities in progress.
The purpose of the inspection was to determine
whether activities authorized by the license were conducted safely and in
accordance with NRC requirements.
A significant problem identified in the report is the continuing failure of
some supervisors to implement management's expectations in such areas as
radiological practices and work package documentation.
Management's failure
to correct this issue is a weakness.
Your actions to improve supervisor
performance will be one of the subjects at our next management meeting.
Based on the results of this inspection, certain of your activities appeared
to be in violation of NRC requirements, as specified in the enclosed Notice of
Violation (Notice).
The violation was cited for failure to maintain control
of locked valves on certain safety related systems.
The violation is of
concern because positive control of certain system components is essential to
safe operation.
You are required to respond to this letter and should follow the instructions
specified in the enclosed Notice when preparing your response.
In your
response, you should document the specific actions taken and any additional
actions you plan to prevent recurrence. After reviewing your response to this
