NOTICE OF VIOLATION Commonwealth Edison Company Dresden Nuclear Power Station Dock~t Nos. 50-237;. 50-24~.
License Nos.
DPR~l9; DPR-25 Du~ing ah NRC inspection conducted o~ March 3, through April 10, 1992, a
-vi-olation of NRC requirements was identified.
In accordance with the "General Statement* of Policy and Procedures for NRC Enforcement Action," 10 CFR Part 2, Appendix C (1~92), the violation is listed below:
10 CFR 50 Appendix B, Criterion V, '"Procedures, Instructions, and Drawings,"
. requires, in part, that activities affecting quality shall. be prescr1bed by documented instructions, procedures, or drawings arid shall be accomplished in
- accordance with these* instructions, procedures, or d_rawi ngs.
Contrary-to the above:
- a.
On February. 11, 1992, an operator perfor~ed an activity affecting quality, placement* of a Tock on the electrical breaker for ~ontainment isolation valve 2-13~1-l,* without a documented instruction or pfocedur2.
- b.
As of February 11, 1992, OAP 07-32,_ "Routine Plant Testing Activities,'.'
a procedure g6verning activittes affecting qualjty, did not prescribe.
any measures.to indicate the status of cqmpon~nts tested under its administrative co~trol.
- c.
On April 4, 1992, personnel failed to accomplish an activity affecting quality in accordance with a prescribed procedure in that critical dr~wing M-359 was not updated to r~flect installation of temporary instrumentation within one day of installation as stated ~n OAP 07-04 Step F. L,f ( 5 )..
- d.
As of March 8; 1992, the Unit Operators Daily.Surveillance Log, was
- inadequate in that it failed to prescribe checking of air samples for reactor coolant leakage in accordance with Technjcal Specification 4.6.D, an activity affecting quality; although it was the ~rocedure designated to meet the surveill~nce requirements.

On March 7,* 1992, operators failed to accomplish an activity affecting.
quality in accordance with a prescribed procedure in that*the diesel generator was not opefated loaded for five minutes as required by step 1.7.a of DOS 6600-03, "Bus.Undervoltage And ECCS Int~grated Functional Test For Unit 2/3 Diesel Ge.nerator (Unit 3 Test Only)".
This is a Severity Level IV violation (Supplement I).

This reply should be clearly marked as a "Reply to a Notice of Violation" and *should.include for each violation:
(1) the reason for the violation, or, if contested~ the basis for disputing the violation, (2) the corrective steps that have been taken and the results achi~ved, (3) the correctiv~ steps that will be taken to avoid furth~r violations, and (4) the date when full compliance will be achieved.
If an adequate reply is not received within the time specified in thi.s Notice, an order may* be issued to show cause why the license should not be modified, suspended, or revoked, or why such other action as m~y be proper should not be.
