NRC INTEGRATED INSPECTION REPORT 50-387/99-04 and 50-388/99-04;
AND EXERCISE OF ENFORCEMENT DISCRETION
Dear Mr. Byram:
On April26, 1999, the NRC completed inspections at your Susquehanna
Steam Electric Station
(SSES) Unit 18 2 reactor facilities. During the 6-week period of inspection, your staff operated
both units safely. The resident inspectors reviewed routine activities. Other NRC inspectors
reviewed the root cause analysis for the low pressure coolant injection valve failures, the
radiological controls program, and the inservice inspection program. The enclosed report
presents the results of these inspections.
The inspectors discussed these results with Mr. R.
Saunders at an exit meeting on May 14, 1999.
Based on our review, we concluded that your staff did a thorough root cause analysis of the low
'ressure
coolant injection valve stem failures.
In addition, your staffs self-assessment
of the
station's response to the failures was generally comprehensive and identified important
shortcomings in the station's approach to troubleshooting.
However, your independent safety
engineering group (ISEG) missed opportunities to provide additional insight into station
activities. Specifiically, neither your staff review nor ISEG identified an engineering interface
issue or the non-adherence
to time limits associated with the work authorization. Atthe exit
meeting, my staff discussed these deficiencies with station management and stated that
continued management attention is warranted in this area.
The NRC completed the review ofthe violation that your staff identified involving improper
material use in low pressure coolant injection valve stems.
Since this finding involves a Severity
Level IIIviolation of NRC requirements, it could be considered for escalated enforcement
including a civil penalty.
However, after consultation with the Director, Office of Enforcement
and the Regional Administrator, I have been authorized to exercise discretion to not issue a
Notice ofViolation or a civil penalty. This decision is in accordance with the provisions of NRC
Enforcement Policy Section VII.B.3because:
(1) the violation was identified by your staff who
exhibited a good questioning attitude in responding to abnormal plant equipment indications; (2)
your corrective actions were generally comprehensive and timely; and (3) the initial design error
of specifying the improper stem material occurred more than 10 years ago which is not
reasonably linked to current performance.
The exercise of discretion acknowledges your overall
effort to identify and correct this violation, before the degraded safety system was called upon.

In addition, based on the results of this inspection, the NRC has determined that 2 Severity
Level IVviolations of NRC requirements occurred.
These violations are being treated as Non-
Cited Violations (NCVs), consistent with Appendix C of the Enforcement Policy. These NCVs
are described in the enclosed inspection report and involved main steam isolation valve leak
rate testing and reportability of an inoperability of the residual heat removal system.
Ifyou
contest the violations or severity level of these NCVs, you should provide a response within 30
days of the date of this inspection report, with the basis for your denial, to the Nuclear
Regulatory Commission, ATTN: Document Control Desk, Washington DC 20555-0001, with
copies to the Regional Administrator, Region I; the Director, Office of Enforcement, United
States Nuclear Regulatory Commission, Washington, DC 20555-0001; and the NRC Resident
Inspector at the Susquehanna
Steam Electric Station.
In accordance with 10 CFR 2.790 ofthe NRC's "Rules of Practice," a copy of this letter, its
enclosure(s), and your response willbe placed in the NRC Public Document Room.
