Dear Mr. Byram:
. This letter forwards the SALP Report for Susquehanna Steam Electric Station, Units 1 and 2, for the period of August 6, 1995, through August 16, 1997 (Enclosure 1). This
'ssessment was conducted under the SALP process that was implemented by the Nuclear Regulatory Commission (NRC) in July 1993.
In this process, the NRC rates the performance of licensees in four functional areas:
Operations, Maintenance, Engineering, and Plant Support.
The Plant Support area comprises radiological and effluent controls, chemistry, security, emergency preparedness, fire protection, and housekeeping.
Overall performance of the Susquehanna station was good during this period.
A generally conservative approach w'as taken to operation of the facility. Plant material condition and safety systems reliability were strong and improvements were made in the corrective action process'.
However, performance declined in several areas as reflected by lower ratings in operations, maintenance, and engineering areas.
Performance declines relate mostly to personnel errors and some significant failures to provide adequate management oversight of important plant activities.
Performance in the operations area was rated Category 2. Operations management demonstrated a conservative approach to operation of the plant. In general, operations management responded aggressively to events to ensure operability and reliability of systems.
However, some significant failures of licensed and non-licensed nuclear plant operators, to perform and document required equipment checks and inspections raised fundamental questions about the adequacy of supervisory oversight and communication of management expectations.
In addition, configuration control weaknesses resulted in the misalignment of three safety related systems.
Performance in the maintenance area was rated Category 2.
Management oversight and involvement in response to high profile maintenance activities resulted in good corrective actions.
Safety system reliability and plant material condition continued to be strengths at Susquehanna.

Robert G. Byram equipment challenges including'two reactor scrams and a reactor recirculation system runback.
Corrective actions for minor human performance errors identified during the previous SALP were not effective in precluding these more significant events.
Licensee self assessments provided insights into maintenance problems.
However, the effectiveness of the licensee's corrective actions to address human performance weaknesses is not yet evident.
Performance in the engineering area was rated Category 2.
Engineering organizations generally made conservative decisions and modifications were properly installed and supported by sound analyses.
However, the quality of safety evaluations and operability evaluations remain a weakness that has continued from the last SALP period.
In addition, several longstanding design and licensing basis issues were either not identified and/or properly corrected.
Performance in the area of plant support was rated Category 1.
Overall, the performance in radiological controls was excellent as evidenced by extensive planning and effective implementation of radiological controls for outage work. The environmental monitoring and effluent control programs continued to demonstrate excellent performance.
The security, emergency preparedness, and fire protection programs continue to be effective.
We have tentatively scheduled a management meeting with you at 2:00 p.m. on October 9, 1997, at the Susquehanna site.
The meeting is open to the public. At the meeting, you should be prepared to discuss our assessment and the initiatives you have ongoing or plan to take to address the weaknesses detailed in our evaluation. provides the schedule of NRC inspections of your facility planned for the. next year.
We will inform you of any changes.
The routine resident inspection effort is not included in this schedule.
