NRC INTEGRATED INSPECTION REPORT 50-387/96-09; 50-388/96-09
Dear Mr. Byram:
On September
9, 1996, the NRC completed
an inspection at your Susquehanna
Steam
Electric Station facility. The enclosed report presents the results of that inspection.
During the six week period covered by this inspection, your conduct of activities at the
Susquehanna
facility was generally good.
Several events affected your ability to operate
both units in a steady state fashion.
Licensed operator response
to these events was
determined to be very good and management
response to the events including corrective
action was determined to be aggressive.
With respect to your construction of an Independent
Spent Fuel Storage Installation (ISFSI),
the NRC communicated to you that some of the current design assumptions
required
further NRC review.
Based on an understanding
of the NRC questions,
you chose to delay

concrete pour on certain portions of the ISFSI laydown areas.
Based on the results of this inspection, one apparent violation was identified involving a
single Core Spray test line containment isolation valve that was opened
and deactivated for
24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> in support of preventive maintenance.
You did not take the action required by
Technical Specification (TS) 3.0.3 when you were unable to comply with the Limiting
Conditions For Operation of TS 3.6.3.
By letter dated August 27, 1996, NRC Inspection Report 50-387/96-08 and 50-388/96-08
identified to you four additional apparent violations that were being considered for
escalated
enforcement action by the NRC staff. Three apparent violations were identified
during our review of the "E" emergency diesel generator
(EDG) misalignment event.
They
were for having less than the required number of operable
EDGs, inaccurate electrical
distribution surveillances,
and failure to follow procedures
and administrative programs.
The fourth was related to a non-licensed
operator's failure to follow your administrative
procedures
for controlling the status of equipment associated
with the Standby Liquid
Control System.
No Notice of Violation is presently being issued for these apparent violations.
They are
being considered for escalated
enforcement action in accordance
with the "General
Statement of Policy and Procedure for NRC Enforcement Actions" (Enforcement Policy),

Please
be advised that the number and characterization
of apparent
violations described
above may change
as a result of further NRC review.
A predecisional enforcement conference to discuss these apparent violations has been
scheduled for November 12, 1996, 10:00 a.m., at the NRC Region
I office. The decision
to hold a predecisional
enforcement conference
does not mean that the NRC has
determined that a violation has occurred or that enforcement action will be taken.
This
conference
is being held to obtain information to enable the NRC to make an enforcement
decision, such as a common understanding
of the facts, root causes,
missed opportunities
to identify the apparent violations sooner, corrective actions, significance of the issues and
the need for lasting and effective corrective action.
In addition, this is an opportunity for
you to point out any errors in our inspection report and for you to provide any information
concerning your perspectives
on 1) the severity of the violations, 2) the application of the
factors that the NRC considers when it determines the amount of a civil penalty that may
be assessed
in accordance
with Section VI.B.2 of the Enforcement Policy, and 3) any other
application of the Enforcement Policy to this case, including the exercise of discretion in
accordance
with Section Vll.
You will be advised by separate
correspondence
of the results of our deliberations on this
matter.
No response
regarding these apparent violations is required at this time.
In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of this letter
and its enclosures
will be placed in the NRC Public Document Room (PDR).
