  • Leadtimeforanewshaftwasbeyondthestartdate ofOperatingCycle22.
  • Inpreparationforrefurbishment,pumpwas uncoupledfrommotor,thenrecoupledwhen refurbishmentcouldnotbecompleted.Noadditional maintenancewasperformed.

ChangeinVibrationReadings Engineeringevaluationdeterminedthatthe uncouplingandrecouplingperformedatthebeginning oftherefurbishmentattemptaffectedthevibration readings;however,thisisnotthemostlikelycauseof theincreasedvibrationsidentifiedinNovember2015 andthereforeEntergycannotsaythattheoriginal conditionhasbeenresolved.Duetothecircumstances presentedbythefactthattheHPSIABpumpcouldnot berefurbished,theoriginalconditionthatcausedthe elevatedvibrationhasnotbeencorrected.

  • Reliefwillberequestedtoallowperformanceof theGroupAtestonaquarterlybasisinlieuof theComprehensiveTestforthedurationofCycle 22,essentiallyidenticaltothereliefgrantedfor Cycle21.
  • Datawillbeanalyzedtoensurethatthe performanceisintheexpectedrange,providing assurancethatthepumpwillcontinuetobe operationallyreadyforthedurationofthecycle.
