INSPECTION REPORT NOS. 50-387/93-18; 50-388/93-18

This letter refers to the safety inspection led by Mr. C. Sisco of this office and

Mr. D. Mannai of the Resident Inspector's office, from September 27-October

1, 1993, at

the Susquehanna

Steam Electric Station, of activities authorized by NRC Licenses Nos. NPF-

14 & 22 and to the discussions of our findings with members of your staff at the conclusion

of the inspection.

Areas examined during this inspection are described in the NRC Region I

Inspection Report, which is enclosed with this letter.

Within these areas,

the inspection

consisted of selective review of representative

records, interviews with personnel,

and

observations by the inspectors.

We have a continuing concern that your corrective actions have not been fully effective to

maintain control of valve positions.

In addition, your root cause analysis of mispositioned

valves may not have fully addressed

human performance during your analyses of the events.

The enclosed report contains the details of our continuing concern.

We request that you respond, in writing, within 60 days of your plans to effect corrective

actions to assure correct valve positions based on sound root cause analysis and conclusions.
