NOTICE OF VIOLATIONAND NRC REGION I COMBINED
INSPECTION 50-387/93-07; 50-388/93-07
The enclosed report documents a resident safety inspection conducted by Messrs.
G. S.
Barber and D. J. Mannai, and others from March 30, 1993 through May 10, 1993, at the
Susquehanna
Steam Electric Station (SSES), Salem Township, Pennsylvania.
The inspection
covered plant operations,
surveillance,
and maintenance for both units.
At the conclusion of
the inspection, Mr. G. S. Barber, Senior Resident Inspector, summarized
the inspection
findings with Mr. H. G. Stanley and other members of your staff.
Areas examined during the inspection are important to public health and safety and are
identified in the report.
Within these areas,
the inspection consisted of selective
examinations of procedures
and representative
records, interviews with personnel,
and
observation of activities in progress.
The purpose of the inspection was to determine
whether NRC licensed activities were being conducted safely and in accordance with
regulatory requirements.

Based on the results of this inspection, certain activities appeared
to be in violation of NRC
requirements,
as specified in the enclosed Notice of Violation (Notice).
The inspector
discovered that a high pressure coolant injection system containment boundary valve was
closed, but not locked, as required by the system checklist.
This valve could allow
communication of primary and secondary containments.
Strict administrative controls
governing valve positioning were not adhered to in this case.
We also noted potential
violations relative to (1) the previous selection of overvoltage setpoint established for the
reactor protection system in the 1982-1988 period, and (2) a matter involving missed rounds
by assigned firewatch personnel in 1990.
However, we intend to execise enforcement
discretion in these matters, and not cite these instances
since you met the criteria specified in
Section VII.Bof our enforcement policy.
You are required to respond to this letter and should follow the instructions specified in the
enclosed Notice when preparing your response.
In your response,
you should document the
specific actions taken and any additional actions you plan to prevent recurrence.

reviewing your response
to this Notice, including your proposed corrective actions and the
results of future inspections,
the NRC will determine whether further NRC enforcement
action is necessary
to ensure compliance with NRC regulatory requirements.
In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of this letter and
its enclosures will be placed in the NRC Public Document Room.
The responses
directed by
this letter and the enclosed Notice are not subject to the clearance procedures of the Office of
Man'agement and Budget as required by the Paperwork Reduction Act of 1980, Public Law
No. 96.511.
