COMBINED EXAMINATIONREPORT NO. 50-387/92-18 AND 50-388/92-18
(OL) AND NOTICE OF VIOLATION
During the week of July 6, 1992, the NRC administered initial examinations and
requalification retake examinations to employees of your company for the Susquehanna
Steam
Electric Station.
The licensed operator training program is one of the key elements that
ensures the continued health and safety of the general public.
The examinations were
administered in accordance with NUREG-1021, "Operator Licensing Examiner Standards,"

6. At the conclusion of the examinations,
an exit meeting was held with members
of your staff identified in the enclosed report.
The test results indicate that all four of the applicants passed the initial examination.
All
three operators passed
the retake requalification examination.
Section 4.0 of the report documents the NRC staff review of your medical records for
licensed operators.
Based on the results this review, certain activities appeared
to be in
violation of the NRC requirements,
as specified in the enclosed Notice of Violation. The
results of the licensed operator medical records review were discussed with Mr. A. Etzel at
the conclusion of the review at the corporate offices on July 21, 1992, and with Mr. H.
Palmer via telephone on July 23, 1992.
The violation is of concern because of our need to
be informed of conditions that may be adverse to safety so that appropriate regulatory
controls can be instituted.
You are required to respond to this letter and should follow the
instructions specified in the enclosed Notice when preparing your response.
In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of this letter and
its enclosures willbe placed in the NRC Public Do'cument Room.
The responses
directed by
this letter and the enclosed Notice are not subject to the clearance procedures of the Office of
Management and Budget as required by the Paperwork Reduction Act of 1980, Pub. L. No.
96.511.
