SUBJECT:
INSPECTION NOS. 50-387/90-81; 50-388/90-81
This refers to your response to our Notice of Violation (NOV) associated
with the subject
inspection report which was undated, but notarized on March 28, 1991
This letter also
acknowledges receipt of your letter dated December
12, 1991, which updates your letter of
April 29, 1991.
The latter letter was in response
to the weaknesses
identified by the subject
inspection,
In reference to your letter dated December
12, 1991, you advised us of your revision to the
estimated completion dates for the I&C Planning Guideline and the I&C Procedure Writer'
Guide.
We find this change in the schedule which was originally provided in your
April 29, 1991, response to be acceptable.
In your letter responding to our NOV, you agreed with Violation Numbers
1, 2, 3, and 5,
and example "a" of Violation Number 4. The corrective actions willbe examined during a
future inspection of your licensed program.
You did not agree with example "b" of Violation Number 4 because it was not an indication
of inadequate work planning.
You stated that there was recognition of the leakage potential
of the butterfly valve HV-11024B1, since the leakage would be into a nonsafety system..Our
inspector was observing the installation work on the valves 110060, 110061, and 110062
when the flooding occurred.
During this work, he observed the worker response
to quickly
complete the flange make-up for the 110060 valve in order to stop the flooding. This
reaction resulted in damage to the valve's flange gasket, which was subsequently
replaced.
Our inspector also noted that the workers were unaware of the potential for flooding when an
emergency service water pump was started to perform testing for another work activity.

The planning process had provided no precautions on the work authorization or Equipment
Release Form (ERF).
The ERF merely stated, "Sign on System Permit 1-90-1033."
Based
upon the apparent lack of coordination between the two jobs, we concluded that'work
planning and instructions to the workers were inadequate,
and the intent of ANSI 18.7-
1972/ANS-3.2 was not met.
Thus, we conclude that Violation No. 4, example "b," should
stand.
Thank you for informing us of the corrective and preventive actions documented in your
letter.
