NRC Region I Combined Inspection 50-387/91-02; 50-388/91-02
This letter refers.,to the inspection conducted by Mr. Scott Barber and other inspectors of this
office from February
12 through March 25, 1991, at the Susquehanna
Steam Electric Station,
Salem Township, Pennsylvania.
Areas examined during the inspection are described in the
enclosed report, and include plant operations,
maintenance
and surveillance associated with
normal and outage activities.
At the conclusion of the inspection, Mr. Barber summarized
the inspection findings with Mr. Stanley and other members of your staff,
No violations of NRC requirements were identified during the course of this inspection.
From our review, your management
and control of activities affecting safety was effective.
However, we have noted an example of an instrumentation installation and/or design error
that was not determined during the design review process or during post-modification testing.
This example involved the determination that the inlet and outlet temperature setpoints
affecting the operation of the "A" Standby Gas Treatment System on March 9, 1991, were
improperly established.
Previously, in 1988, Main Steam Isolation Valve differential
temperature detectors were found to been miswired.
While the occurence of this type of
finding appears infrequent, it highlights the importance of ensuring your design review and
post-modification testing programs can detect similar deficiencies during the modification
pi'ocess.
No response
to this letter is required.
In accordance with 10 CFR -2.790 of the NRC's
"Rules of Practice,"
a copy of this letter and the enclosure willbe placed in the NRC Public
Document Room.
