NOTICE OF VIOLATIONCombined Inspection 50-387/90-26; 50-388/90-26

The enclosed report documents a routine safety inspection conducted by inspectors of this

office from December 30, 1990 to February 11, 1991, at the Susquehanna

Steam Electric

Station (SSES), Salem Township, Pennsylvania.

The inspection covered plant operations,

surveillance, 'and maintenance activities for both units.

At the conclusion of the inspection,

Mr. G. S. Barber, NRC Senior Resident Inspector, summarized the inspection findings, as

detailed in the enclosed report, with Mr. H. G. Stanley and other members of your staff.

Based on the results of this inspection, it appears

that one of your activities was not in full

compliance with NRC regulations.

On January

15, 1991, the inboard Main Steam Isolation

Valves (MSIVs) for Susquehanna

Unit 1 were declared inoperable due to low, Containment

Instrument Gas (CIG) Pressure.

However, the plant operators failed to recognize that

Technical Specification 3.6.3 applied as a Limiting Condition of Operation in this instance;

and the Shift Surveillance Operating Log Procedure SO-100-006 failed to include any

reference to this Technical Specification.

Accordingly, this matter is identified as a violation

of Technical Specification 6.8.1, relative to failure maintain plant conditions within the

boundaries specified by the Technical Specification and License Conditions as required by

Procedure 80-QA-300, Conduct of Operations.

This issue is set forth in the Notice of

Violation enclosed

as Appendix A. You are required to respond to this letter and should

follow the instructions specified in the enclosed Notice when preparing your response.

In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of this letter and

its enclosures will be placed in the NRC Public Document Room.

The responses

directed by

this letter and the Notice are not subject to the clearance procedures of the Office of

Management and Budget as required by the Paperwork Reduction Act of 1980, Pub. L. No.96-511.
