NOTICE OI" VIOLATION

INSPECTION REPORT NOS. 50-387/90-81; 50-388/90-81

This letter refers to the special maintenance

team inspection led by Mr. D. Caphton of this office

from October 9 to October 19 and November 5 to November 9, 1990.

The inspection included a

review of activities authorized for the Susquehanna

Steam Electric Station at Berwick,

Pennsylvania.

At the conclusion of the initial inspection on October 19, 1990, Mr. Caphton

discussed

the team's findings with you and those members of your staff identified in the enclosed

report.

This inspection was performed as part of the NRC's industrywide effort to evaluate the

effectiveness of maintenance activities at licensed power reactors.

Areas examined during this

inspection are identified in the enclosed report.

The team concluded that the maintenance program

at the Susquehanna

Steam Electric Station is acceptable.

However, based on the results of this inspection, certain activities appeared

to be in violation of

NRC requirements,

as specified in the enclosed Notice of Violation (Notice).

We are concerned

about the violations because of the possible safety hazards which could result from the existence of

potentially unreviewed safety questions,

the failure to maintain environmental qualifications, the

lack of independent review of plant procedures,

the failure to adequately preplan m'aintenance

activities, and the lack of regard for procedural controls.

In your response,

you should document the

specific actions taken to prevent recurrence.

After reviewing your response to this Notice,

to ensure compliance with NRC

Further, we ask you to notify us, in writing, of planned and completed, corrective actions taken to

improve your maintenance program in the areas where weaknesses

were identified.

Please send us

this response within 60 days of receiving this letter.

In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of this letter and its

enclosures willbe placed in the NRC Public Document Room.

The responses

directed by this letter and the enclosed Notice a'e not subject to the clearance

procedures of the Office of Management and Budget as required by the Paperwork Reduction Act

of 1980, Pub. L. 96.511.
