The enclosed
report documents
a routine safety inspection
conducted
by
inspectors
of this office on September
10,
1989
October
14,
1989, at the
Susquehanna
Steam Electric Station
(SSES),
Salem Township,
The,
inspection
covered plant operations,
surveillance,
and maintenance
for both
units.
At the conclusion of the inspection,
Mr.
G. Barber
summarized
the
inspection findings to Mr. R.
Byram and other
members of your staff.
The
NRC reviewed your response
and corrective actions related to the diesel
failures that occurred
on September
16 and October
8.
We noted that you took
immediate actions to replace
the
damaged
pistons
and cylinder liners-and
focused your attention
on promptly restoring both the "B" and "C" diesel
generators
to an operable
status.
We also reviewed your plans to determine
.the root cause of these failures.
The
NRC is concerned that the cause
of the
crankcase
vapor ignitions may be due to
a generic failure mechanism
in the
Cooper-Bessemer
diesels.
We encourage
you to expedite your root cause
identification and corrective action determination
and to inform us in
writing of the results.
