A routine radiological controls inspection
was conducted
by Mr.
R.
L. Nimitz
at the Susquehanna
Steam Electric Station,
Units
1 and 2,
on
October 16-20,
1989.
The inspection concentrated
on reviews of radiological
controls during the Unit 2 outage'he
findings'of the inspection
were
discussed
with Mr. R. Byram.
Two apparent violations were identified and are discussed
in the enclosed
The first apparent violation involved failure to adhere
to radiation protection procedures'he
second
apparent violation involved
failure to maintain records of radiation
surveys.
You are required to respond
to the enclosed
Notice of Violation and should follow the instructions
contained therein.
The inspector also identified
a problem involving lack of'adherence
to your
confined
space
entry procedures.
Although your staff took corrective actions
when this matter was brought to their attention,
the failure of personnel
to
adhere
to important safety procedures
is
a concern to the
NRC.
Additional
management
attention to this area
may be warranted.
We also identified concerns
regarding
the maintenance
and care of control
structure ventilation systems.
Your attention is directed to the
NRC
unresolved
items identified in the enclosed
report.
These findings and other
findings detailed in the enclosed report indicate
an apparent
need for
enhanced attention to detail
by personnel.
