APPENDIX A NOTICE OF VIOLATION Pennsylvania Power and Light Company Susquehanna Unit 2 Docket No. 50-388 License No. NPF-22 During an NRC inspection conducted on July 16 September 1,

1986 a violation of NRC requirements was identified.

In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions,"

10 CFR Part 2, Appendix C (Enforcement Policy 1986),

the violation is listed below:

Technical Specification Limiting Condition for Operation 3.6.3 states that the primary containment isolation valves shall be operable in Operational Conditions 1,

2 and 3.

With one or more of the primary containment isolation valves inoperable, at least one isolation valve is to be maintained operable in each affected penetration that is open and within 4 hours4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br /> the inoperable valve is to be restored to operable status or the affected penetration is to be isolated.

Otherwise, the reactor is to be in at least Hot Shutdown within the next 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br /> and in Cold Shutdown within the following 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />.

Contrary to the above, between 4:25 p.m. July 16, 1986 and 7:18 p.m. July 17, while in Operational Condition 1, the primary containment isolation valve on the 'C'raversing Incore Probe tube was inoperable, in that the TIP drive mechanism was deenergized with the TIP inserted in containment, and the penetration was neither restored nor isolated within 4 hours4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br />.

In

addition, the reactor was not in Hot Shutdown within the next 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />.

This is a Severity Level IV Violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Pennsylvania Power and Light is hereby required to submit to this office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including:

( 1) the reason for the violation if admitted; (2) the corrective steps which have been taken and the results achieved; (3) the corrective steps which will be taken to avoid further violations; and (4) the date when full compliance will be achieved.

Where good cause is shown, consideration will be given to extending this response time.
