APPENDIX A NOTICE OF VIOLATION Pennsylvania Power and Light Company Docket Nos.
50-387 50-388 This refers to the inspection conducted by a representati,ve of the Region I (Philadelphia) office at the Susquehanna Steam Electric Station, Salem Township, Pennsylvania, of activities authorized by NRC License Nos.
CPPR-101 and CPPR-102.
During this inspection conducted on September 17-25, 1980, the following item of noncompliance was identified.,
A.
10 CFR 50, Appendix B, Criterion V, states, in part; "Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and...shall include appropriate quantitative or qualitative acceptance criteria...".
The Susquehanna PSAR Appendix D.2.5, states, in part, "Activities affecting quality will be prescribed by documented instructions, procedures, or drawings appropriate to the circumstances...".
.Section 16.1.2 of Specification 8856-C-8, states, in part, "All fillings shall be bonded tightly to the surface of the holes, shall be sound and free from shrinkage cracks and hollow. sounding areas...".
Contrary to the above, during September 17-25, 1980, field procedure FP-N-1 was, in use which did not contain the required quantitative or qualitative acceptance criteria.
As a result, post equipment turnover repair of g-listed grouting was necessary to extend equipment warranty.
This is an infraction.
