APPENDIX A NOTICE OF VIOLATION Pennsylvania Power and Light Company Do'cket Nos.
50-387 50-388 This refers to the inspection conducted by a representative of the Region I (Philadelphia) office at the Susquehanna Steam Electric Station, Berwick, Pen-nsylvania, of activities authorized by. the-NRC License Nos.
CPPR-101 and CPPR-102.
During this inspection, conducted on April 1 through 4, 1980 and April 9 and 10, 1980 the following apparent items of noncompliance were identified.
These items are infractions.
A.
10 CFR 50, Appendix B, Criterion XIII states, in part, that:
"Measures shall be established to control...cleaning and preservation of material and equipment...to prevent damage and deterioration...."
PSAR Section D.2.5 states, in part, that:
"Activities affecting quality will be prescribed by documented instructions...appropriate to the circum-stances."
'ection 3.3.6 of Bechtel Field Procedure FP-G-11 states, in part, that:
-"... switchgear and other electrical components shall be protected and maintained during lay-ups as required by Vendor Instructions."
Reference 5 of Vendor Specification No. GE-ZZA2724, Revision 3, states, in part,'that:
"...control and instrumentation supplied by GE...shall be covered and sealed as necessary to protect against dirt, dust and other airborne contaminants...."
Contrary to the above, as of April 2, 1980, measures had not been established to control or prevent damage/deterioration to the interconnecting cable as-sembly connectors of the safety-related control rod drive installation in Unit 2.
B.
10 CFR 50, Appendix B, Criterion II, states, in part, that:
"...the quality assurance program shall provide control over activities affecting quality of...systems....
Activities affecting quality shall be accomplished under controlled conditions.
Controlled conditions include the use of appropriate equipment....
The program shall take into account the need for special controls...test equipment, tools and skills to attain the required quality and need for verification of quality by inspection and test;..."
PSAR Section D.2.2 of Appendix D states, in part, that:
"The guality As-surance Program for the Susquehanna Steam Electric Station, Units 1 and 2, will be responsive to,"and meet the intent of 10 CFR 50, Appendix B.

APPENDIX A Contrary to the above, on April 4, 1980, guality Control personnel were observed measuring the slope of instrument tubing runs in Unit, 1 in a manner that did not result in a true determination of the line slope.
The method and tool used were inadequate and slope measurements were shown to be in error by as much as fifty percent for instrument lines from panel 1C017.

10 CFR 50, Appendix B, Criterion VIII, states, in part,.that:
"...measures shall be established for the identification',and control of...components in-cluding partially fabricated assemblies....
These identification and con-trol measures shall be designed to prevent the use of incorrect...materials, parts and components."
PSAR Section D.3.3.6 states, in part, that:
"...Bechtel field procedures and practices incorporate measures for material control...."
Contrary to the above, on April 4, 1980, a tubing support bracket, type FI-25, had been installed in Unit 1 to support thermoelement.

TE-1N023A instead of the Class'E instrument support bracket'specified in thermo-element Support Drawing, ZE-53, sheet 812.
r 10 CFR 50, Appendix B, Criterion Y, states, in part, that:
"Activities af-fecting quality shall be prescribed by documented instructions...and shall be accompli shed in accordance with these instr uctions.... "
PSAR Section D.2.5 states, in part, that:
"Activities affecting quality will be prescribed by documented instructions, procedures...."
Section 3.3.6 of Bechtel Field Procedure FP-G-11 states, in part, that:
";.. switchgear and other electrical components shall be protected and maintained during lay-up as required by...Vendor Instructions."
Reference 5 of Vendor Instructions, Specification No.
GE-22A2724, Revision 3, states, in part, that:."...Control and instrumentation supplied by GE...shall be stored inside heated (IH) and shall be covered and sealed as necessary against dirt, dust or other airborne contaminants....
If during in-plant storage or final location storage the heating and ventilation conditions are not adequate to ensure that condensation cannot occur...,
condensation controls...must be employed."
Contrary to the above, as of April 10, 1980, condensation/temperature con-trols have not been implemented in the Unit -2 upper/lower relay rooms.
