This serves to confirm information provided to NRC Resident Inspector R.. Gallo on 2/27/79 and relates to the subject deficiency which has been determined to be reportable. under the provisions of 10CFR50.55(e).
PPGL'as been advised by Bechtel Corporation's quality assurance staff organization that a generic problem exists with regard to the failure of valve manufacturers to stake the stem nuts fastening certain Limitorcye motor operatotrs to valves.
This condition has been identified with respect to Limitorque Model No.
SMB and Model No.
SMC motor operators.
f The potential for impact on safety has been established since the failure to stake the lock nut may result in the operators of their respective valves to cease operating at any valve position.
In assuming a worst case condition for safety related systems, failure of a valve operator to properly acctuate its respective valve could affect the safe shut down of the plant.
Bechtel's Quality Assurance Supervisor for the SSES Project has, through MCAR-1 G-l, directed that a plan for investigation and corrective action be devised for SSES.
