MAY 19 1980 APPENDIX A Pacific Gas and Electric Company 77 Beale Street San Francisco, California 94106 Docket No. 50-275 Construction Permit No.

CPPR-39 Notice of Violation Based on the results of an NRC site inspection conducted during the period of April 29 through May 2, 1980, it appears that one of your activities was not conducted in full compliance with Construction Permit No.

CPPR-39, as indicated below.

10 CFR 50, Appendix B, Criterion V, as implemented by Section

17. l. 5 of the FSAR and the PGSE guality Assurance Manual Section V. states in part that, "Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings."

The "Pacific Gas and Electric Company Response to NUREG-0578:

Short Term Lessons Learned Requirements" in Section

2. 1. 1 commits to the installation of an emergency power supply for the pressurizer heaters.

Design Change Order No.

DCO-E-E-417, implementing the above commitment requires that the installation be accomplished as specified by, among

others, Drawing No.

57601.

Drawing No.

57601 (Cable Tray and Conduit Layout Plan Below Elevation 115', Area H) prescribes that conduit Nos.

K7107, K7108, K7109 and K7110 be supported by Class IE supports.

H. P. Foley Company quality control procedure No. gCP-9 (guality Control Procedure for Installation of Raceways, Junction and Terminal Boxes for Mire Pull) prescribes in paragraph 4.18 that "an inspection of each class 1 installation listed shall be initiated" upon receival of the conduit installation form by guality Control and,further requires that "each inspection shall be documented on the appropriate inspection report."

gCP-9 further requires in paragraph 5.4, that "Class IE supports requiring inspection shall be documented on Form HPF/SIMS."

Contrary to the above requirements, the NRC inspector observed on May 2, 1980, that the following conduit supports had not been inspected and documented by the H.

P. Foley Company.

1.

Four supports in the vital 480 V switchgear room 1G and six other supports in an adjacent ar ea for conduit K7107,

K7108, K7109 and K7110.

2.

Support Hos.

H-115-4-145 and 146 in the 480 volt vital switchgear room IH.

This is a deficiency.
