Docket No. 50-220
Mr. B. Ralph Sylvia
Executive Vice President - Nuclear
Niagara Mohawk Power Corporation
301 Plainfield Road
Syracuse,
New York 13212
Dear Mr. Sylvia:
SUBJECT:
EXAMINATIONREPORT NO. 50-220/93-26 (OL)
During the week of November 15, 1993, the NRC administered initial examinations to
employees of your company who had applied for licenses to operate the Nine Mile Point
Nuclear Station, Unit 1. At the conclusion of the examinations,
the preliminary findings
were discussed
with those members of your staff identified in the enclosed report.
Four of the six applicants passed
the initial examinations.
Section 3.0 of the enclosed report
contains additional details of the applicants'erformance
and the NRC
examiners'bservations.
Allof the applicants passed
the dynamic simulator portion of the operating
test; however, the performance of the crews was weak in response
to a loss of all reactor
water level indication.
You are requested
to review the individual examination results
provided to Mr. R. Smith in a letter dated December 21, 1993, for generic training
weaknesses.
After your review is complete, we would like to meet with you to discuss the
results of your review.
Several minor mistakes were made by an applicant and NRC examiners during administration
of the examinations that resulted in failure to follow the Radiation Work Permit (RWP)
procedure.
'Based on the prompt identification by the licensee and immediate corrective
action, the failure to follow procedures
was considered
a non-cited violation.
In accordance with 10 CFR 2.790 of the Commission's regulations,
a copy of this letter and
its enclosure will be placed in the NRC Public Document Room.
