INSPECTION REPORT NO. 50-410/92-27

This letter refers to the safety inspection led by Mr. J. H. Williams of this office from
September
14 - 18, 1992, at Nine Mile Point, Unit 2, of activities authorized by NRC
License No. NPF-69 and to the discussions of our findings with members of your staff at the
conclusion of the inspection.
Areas examined during this inspection are described in the
NRC Region I Inspection Report which is enclosed with this letter.
Within these areas, the
inspection consisted of selective review of procedures
and representative
records, interviews
with personnel,
and observations by the inspectors.
The follow-up was necessary
(and will
continue) because it addressed
your resolution of safety significant findings and conclusions
noted by the NRC Incident Investigation Team for the loss of power incident on
August 13, 1991

Overall, your corrective actions in response to the IIT report were appropriate,
and you met
your commitments.
This inspection verified certain actions as described in your letter of
May 11, 1992, to Mr. Hodges that you have taken in response to the findings and
conclusions of NUREG-1455, "Transformer Failure and Common-Mode Loss of Instrument
Power at Nine Mile Point, Unit 2, on August 13, 1991."
We continue to have concerns over
the adequacy and use of procedures at Nine Mile Point, Unit 2.
Based upon this inspection,
procedure adequacy is an unresolved item.
The use of procedures during the
August 13, 1991, transformer failure event is also unresolved.
We will follow up on these
issues in the future.
