NOTICE OF VIOL'ATION
(NRC COMBINED INSPECTION NOS. 50-220/92-17; 50-410/92-19)
This refers to the safety inspection conducted by Mr. J. Furia of this office on June 15-19,
1992, of activities authorized by NRC License Nos. DPR-63 and NPF-54, and to the
discussion of our findings held by Mr. J. Furia with you and other members of your staff at
the conclusion of the inspection.
Areas examined during this inspection are important to health and safety and included
radiological controls during the maintenance
outage at Unit 1, and during restart operations
at Unit 2, dosimetry records,
and transportation
as described in the NRC Region I Inspection
Report which is enclosed with this letter.
Within these areas,
the inspection consisted of
selective examinations of procedures
and representative
records, interviews with personnel,
and observations by the inspector.
Radiological controls in use at Unit 1 during the maintenance
outage were determined to be
very good.
Improvements in ALARAand radiological housekeeping
were noted at Unit 2.
However, based upon this inspection, it appears
that during the recent Unit 2 refueling
outage certain of your activities were not conducted in full compliance with NRC
requirements,
as set forth in the Notice of Violation, enclosed herewith as Appendix A.
Specifically, during the reactor reassembly,
a contractor engineer instructed two contractor
workers to perform wire brushing operations in a manner inconsisterit with the Radiological
Work Permit (RWP) being utilized, and one of the contractor workers was not wearing the
appropriate protective clothing as specified in the RWP.
You"are required to respond to this
letter and should follow the instructions specified in the enclosed Notice when preparing your
response.
In your response,
you should document the specific actions taken and any
additional actions you plan to prevent recurrence.
