NRC Region I Combined Inspection Report Nos. 50-220/91-12 and 50-410/91-12
This refers to the results of the resident safety inspection conducted by Messrs. W. Schmidt,
R. Laura, and R. Temps from June 23 to July 27, 1991, at Nine Mile Point Units
1 and 2,
Scriba, New York. The inspection findings were discussed with members of your staff at the
conclusion of the inspection.
This inspection was directed toward areas important to public health and safety as identified in
the enclosed
inspection
report.
Within these
areas,
the inspection
consisted of selective
examinations
of procedures
and
representative
records,
interviews
with personnel,
and
observation of activities in progress.
Overall, performance in the areas
reviewed was generally good.
However, our inspectors
identified instances of inadequate control of temporary modifications at both units, as described
in section 7.3 of the enclosed inspection report.
Temporary blowers were installed to cool two
fire panels in the reactor building at Unit 1 and mechanical flushing water jumpers were installed
on the service water radiation monitors at Unit 2. In both instances,
the alterations were made
without adherence
to the temporary modification program.
The safety significance of these
issues was minor.
Nonetheless,
we are concerned
about the failure of your staff to properly
implement these alterations to plant equipment,
as well as apparent weaknesses
in addressing
past problems in this area.
This issue is considered
an unresolved item, pending further NRC
review of the NMPC temporary modification process.
However, this matter is brought to your
attention for your information and consideration of actions
as you deem appropriate.
