NRC Combined Inspection Nos. 50-220/91-24 and 50-410/91-24

This refers to the results of the resident inspector

safety inspection

conducted

by Messrs.

W. Schmidt, R. Laura, and R. Temps from November 10 to December 21, 1991, at Nine Mile

Point Units

1 and 2, Scriba, New York. To allow further evaluation of two Unit 2 events, the

inspection period was continued from December 22 through January 4, 1992.

The inspection

findings were

discussed

with members

of your staff at the exit meeting

conducted

on

January

13, 1992.

This inspection was directed toward areas important to public health and safety.

Areas examined

during the inspection are described in the enclosed inspection report.

Within these areas,

the

inspection consisted of observation of activities in progress,

interviews with personnel,

and

selective examinations of procedures

and representative

records.

Performance at both units was generally good.

However, in one instance Unit 2 maintenance

personnel failed to adhere to a station procedure, resulting in damage to two new (non-irradiated)

fuel bundles.

Our review of this event was extensive,

as described in the enclosed

inspection

report.

The cause of the damage was failure to follow the new fuel inspection procedure,

thus

allowing the bundles to drop while being transported to the new fuel inspection stand.

We are

concerned with this event because the identified contributing causes were poor worker practices,

a lack of supervision for the specific job', and ineffective management actions to correct previous

inattention-to-detail issues,

as described in previous inspection reports.

This issue is considered

an apparent violation.

Based

on the results of this inspection,

and on your recent performance

with respect

to

inattention-to-detail

issues,

the apparent

violation discussed

above is being considered

for

escalated enforcement action, in accordance with the "General Statement ofPolicy and Procedure

for NRC Enforcement Actions" (Enforcement Policy), 10 CFR Part 2, Appendix C (1991).

Accordingly no Notice of Violation is presently being issued for this inspection finding. Please

be advised that the number and characterization of apparent violations described in the enclosed

inspection report may change as a result of further NRC review.

are to discuss the apparent violation, its

causes

and safety significance; to provide you the opportunity to point out any errors in our

inspection report; to provide opportunity for you to present your proposed corrective actions;

and to discuss any other information that will help us determine the appropriate enforcement

action in accordance with the Enforcement Policy. In particular, we expect you to address your

overall corrective actions and why previous actions were not fullyeffective in minimizing events

resulting from inattention-to-detail.

You will be advised by separate

correspondence

of the

results of our deliberations on this matter.

regarding this apparent violation is

required at this time.
