Anchor Bolts.

The Bulletin was issued to address

problems at nuclear

plants regarding

concrete

anchor bolts.

The inspector

reviewed the licensee's

response

to the Bulletin and ver-

ified, by reviewing documentation,

gC records,

and by measuring

and exam-

ination of selected

installed bolts, that the actions required

by the

Bulletin were met and that the bolt marking system accurately identified

the bolt length.

Based

on the above, this item is considered

closed.

(Closed) Violation (410/86-13-03):

Pipe supports with no measurable

gap.

The

NRC found two Scram Discharge

Header

supports,

lA and

11A, had

no

measurable

gap.

The licensee's

response

to the violation reported that

a

surveillance

performed

by

SWEC identified a third support,

18B, with a

similar problem.

The licensee further reported that the three

supports

were repaired

and the deficient gaps

were corrected.

The inspector

reviewed the licensee's

response

to the violation,

gC re-

cords

and,

in addition,

independently

measured

the

gap in each of the

three

supports

to ascertain

that the repairs

were effective.

He found

that the originally deficient gaps

were properly repaired

and meet appli-

cable

gap requirements.

Based

on the above this item is considered

closed.

(Closed) Violation (410/86-13-01):

Rigging of equipment to safety related

components.

The

NRC finding involved two instances

of equipment being

improperly rigged to safety related

components

in the plant.

The inspec-

tor verified, through

a review of records,

that the corrective

and pre-

ventive actions

by the licensee

were carried out.

He found that the re-

quired evaluation

was made,

the applicable

procedure

was revised

and per-

sonnel

were issued

reminders

regarding rigging requirements

at the plant.

on the

above this item is considered

closed.

Item (410/86-13-04):

Absence of damper counterweights

on backdraft

damper

2HVC 'OMP 63.

The inspector

reviewed licensee

records

which indicate that .he damper

operates

satisfactorily with the counterweight

removed.

Included in the

inspector's

review was

a letter dated

Nay 27,

1986,

from the equipment

manufacturer,

which states

that in small dampers,

usually those

less than

one square

foot in surface

area,

the counterweight is not absolutely

ne-

cessary

to assist

in blade closure.

The damper in question is 6" x 6" or

'4 square foot in surface

area.

Based

on the

above this item is considered

closed.

(Closed) Violation (410/86-08-01):

Drawing in use without having the re-

quired verification stamp.

The inspector

reviewed licensee

records which

indicated that the drawing in question

was found to be the latest issue

and correct for use.

Additionally gA surveillances

found

no further

examples of unstamped

drawings in use in the plant.

The inspector

found

that the appropriate

personnel

were reminded of the requi rement for draw-

ings to be properly stamped,

initialed and dated prior to use.

Based

on the above this item is closed.

The inspector

met with licensee

representatives

(denoted

in paragraph

1)

at the conclusion of the inspection

on August 1,

1986.

The inspector

summarized

the purpose

and the

scope of the inspection

and the findings.

At no time during this inspection

was written material

provided by the

inspector to the licensee.
