APPENDIX A NOTICE OF VIOLATION Niagara Mohawk Corporation Docket No. 50-220

'I Based on the results of an NRC inspectiqn conducted on September 15-19, 1980, it appears that certain of your activities wer e not conducted in full compliance with the conditions of your NRC license as indicated below.

Item A is an infraction and item B is a deficiency:

A.

Technical Specifications 6.8.1 requires, in part, that "Written procedures

... shall be established, implemented and maintained..."

Plant procedure APN-8, "Test and Inspection Program",

paragraph 6.2.3

requires, in part, that "... When a test is satisfactorily completed the supervisor will... forward to the Test ahd Inspection (TSI) Coordinator the completed data sheets for review and analysis.

The Test and Inspection Coordinator will forward completed data sheets to the Station general files."

Contrary to the above, the following are examples of unsatisfactory test results'hat were being forwarded to the TSI Coordinator.

Additionally, the TSI review and analysis of test results were incomplete in that they were being forwarded to the station general files without the uinsatisfactory results being identified or corrected (or evaluated to determine. acceptability):

Procedure NI-ISP-IC-6, "Off-Gas Line Radiation Monitor Instrpent Channel Calibration," step,5.11.2, requires a meter indication of 10 for Hi Cal adjustment.

Channels 11'nd 12 meter indications were recorded as

]0> for calibrations performed April 28, 1980.

Procedure NI-ISP-NEU-2, "Intermediate Range Monitor Instrument," step 5.7.1 requires the'IRM Upscale Hi Hi reading to be less than or equal to 120 percent power.

IRM Channel 17 upscale Hi Hi trip set point was recorded as 125 percent power for calibration performed July ll, 1980.

'rocedure No. NI-ICP-48, "Control Rod Drive - Accumulator Pressure and Level," step 5.2.3, requires that the low pressure alarm be set at or above 970 psig decreasing pressure.

For calibrations performed on June 18, 1979, nine of 124 low pressure alarm set points calibrated were left at less than 970 psig.

Procedure No. NI-ICP-41, "Liquid Poison System,"

step 5.3.1, requires calibration of the liquid poison system pressure transmitter and indi-cators in the range of 0-1500 psi.

For calibrations performed January 23, 1980, the local pressure indicator had been calibrated in a range of 0-300 psi.

Appendix A Technical Specification,6.10.1 states in part, "The following records shall be retained at least five years:...d.

Records of surveillance activities, inspections and calibrations required by these Technical Specifications..."

Contrary to the above, the following records of surveillance calibrations required by the Technical Specifications could not be located during the inspection and were, apparently not retained:

Procedure NI-ISP-12, Condenser Low-Low-Low Vacuum Instrument, for calibration performed June 12, 1979.'rocedure NI-ISP-XMTR, Reactor Protection System Transmitter Calibration associated Data Sheet 01-26A-H, Main Steam High Flour Containment Isolation Signal, for calibration performed January 1980.
