'APPENDIX A NOTICE OF VIOLATION Niagara Mohawk Power Corporation Docket No. 50-220 Based on the results of an NRC inspection conducted on May 5-9, 1980, it appears that one of your activities was not conducted in full compliance with the con-ditions of your License, DPR-63, as indicated below.
This item is a Deficiency.
Technical Specifications 6.5. 1.6, 6.5. 1.7 and 6.6. 1 require, in part, that the Site Operations Review Committee (SORC) review all proposed changes to the Technical Specifications, and recommend to the General Superintendent, Nuclear Generation,. written approval of the proposed changes; investigate all violations of the Technical Specifications and prepare and forward a report covering evalu-ation and recommendation to prevent recurrence to the Vice President
- Electric Production and to the Chairman of the Safety Review and Audit Board; render determinations in writing with regard to whether or not each violation consti-tuted an unreviewed safety question; review facility operations to detect poten-tial safety hazards; and review each Reportable Occurrence Report submitted to the NRC.
Technical Specification 6.5.1.8 also requires that minutes of SORC meetings be maintained.
Contrary to the above, SORC meeting minutes failed to document the following:

- Change, dated June 27,
- 1979, and, the associated written determination that the change did not constitute an unreviewed safety question.
Investigation of all violations of the Technical Specifications and the preparation and forwarding of a report covering evaluation and recommenda-tions to prevent recurrence to the Vice President - Electric Production and to the Chairman of the Safety Review and Audit Board, and the"associated failure to document SORC resolution and corrective action recommendations
'n eight facility occurrence reports as required by Administrative Proce-dure APN-21.
Review of Licensee Event Report No. 80-01.
Review of facility operations to detect potential safety hazards and the associated failure to hold a special SORC meeting to assess operations experience as required by Administrative Procedure APN-3A.
