SUBJECT:
NRC INSPECTION REPORT 50-275/96-19; 50-323/96-19 AND NOTICE OF
VIOLATION
Dear Mr. Rueger:
A special NRC inspection was conducted August 5-9, 1996, at your Diablo Canyon Power
Plant, Units
1 and 2 reactor facilities. The enclosed report presents the scope and results
of that inspection.

During this inspection, your conduct of,activities at the Diablo Canyon Power Plant facility
~

~
was generally characterized
by safety-conscious
operations.
However, two violations of
NRC requirements were identified.
In the first instance, procedures
controllin'g high voltage
grounding devices (ground buggies) were not followed. We are particularly concerned that
corrective actions implemented in response to the October 1995 failure event of Auxiliary
Transformer 1-1 did not prevent this violation. In the second instance, refueling outage
clearances
were not approved by the shift foreman, as required by administrative
procedures.
Furthermore, the shift foreman's clearance
approval authority was delegated
to another senior reactor operator without documenting the delegation as required by
administrative procedures.
Consequently,
approvals for clearance activities were routinely
executed by an unauthorized person.
These violations are cited in the enclosed Notice of Violation (Notice), and the
circumstances
surrounding the violations are described
in detail in the enclosed report.

instructions specified in the enclosed Notice when preparing your response'.
The NRC will
use your response,
in part, to determine whether further enforcement action is necessary
to ensure compliance with regulatory requirements.
