AND NOTICE OF
VIOLATION
This refers to the inspection
conducted
by J. Dixon-Herrity, J.Russell,
and
S.
Boynton of this office on October
29,
1995 through
December
9,
1995, at the
Diablo Canyon Nuclear
Power Plant,
Units
1 and 2, facility.
The purpose of
the inspection
was to determine
whether activities authorized
by the license
were conducted safely
and in accordance
with NRC requirements.
At the conclu-
sion of the inspection,
the findings were discussed
with those
members of your
staff identified in the enclosed
report.
Areas
examined during the inspection
are identified in the report.
Within
these
areas,
the inspection consisted
of selective
examinations of procedures
and representative
records,
interviews with personnel,
and observation
of
activities in progress.
The results of this inspection
are documented
in the
enclosed
report.
Based
on the results of this inspection,
the
NRC has determined that two
violations of NRC requirements
occurred.
The violations are cited in the
enclosed
Notice of Violation (Notice)
and the circumstances
surrounding
them
are described
in detail in the subject inspection report.
The violations are
of concern,
because
they provide further examples of the failure of personnel
to follow procedures.
The first violation involves
an operator failure to
follow procedures
during
system draindown.
The second
violation involves operators
and maintenance
technicians failure to follow
procedures
(or make changes
to the procedures)
during testing of source
range
nuclear instruments.
The third violation involves the failure of cleaning
personnel
to follow procedures
to notify personnel
of a blocked fire door in
a
diesel
generator
room.
The violations indicate that continued effort is
needed
to assure
the corrective actions
taken after the Unit
1 transformer
fire are fully effective.
You are required to respond to this letter and should follow the instructions
specified
in the enclosed
Notice when preparing your response.
In your
response,
you should
document
the specific actions
taken
and
any additional
actions
you plan to prevent recurrence.
Your response
may reference
or
include previous
docketed
correspondence, if the correspondence
adequately
addresses
the required
response.
After reviewing your response
to this
Notice, including your proposed corrective actions
and the results of future

whether further
NRC enforcement
action is
necessary
to ensure
compliance with NRC regulatory requirements.

To the extent possible,
