This refers to the inspection
conducted
by H. Tschiltz and
G. Johnston of this
office,
and
H. Hiller of the Office of Nuclear Reactor Regulation,
on Hay
14
through June
24,
1995.
The inspection
included
a review of activities
authorized for your Diablo Canyon Nuclear
Power Plant, Units
1
and 2,
facility.
At the conclusion of the inspection,
the findings were discussed
with those
members of your staff identified in the enclosed
report.
Areas
examined during the inspection
are identified in the report.
Within
these
areas,
the inspection consisted
of selective
examinations
of procedures
and representative
records,
interviews with personnel,
and observation of
activities in progress.
The results of this inspection
are documented
on
page
1, in the enclosed
report.
Three noncited violations are included in this report.
The first violation
involved the failure to comply with a maintenance
procedure for maintaining
and monitoring
a freeze seal.
The second violation was for failing to provide
adequate
emergency lighting for safe
shutdown
areas of the plant.
The third
violation involved
a solid state protection
system design deficiency,
discovered
by your staff, which resulted
in inadequate
electrical
separation
between safety-related
and nonsafety-related
circuits.
The actions that were
taken
by your staff to identify and correct this issue
are considered
to have
been noteworthy.
Although this deficiency
had the potential to be of safety
significance,
the probability of the initiating event which could have
potentially caused
the failure was low.
These violations are not being cited
because
the criteria in Paragraphs
VII.B.1 or
2 of Appendix
C to
of the NRC's "Rules of Practice,"
were satisfied.
We note that there were
several
instances
in this inspection period which demonstrated
weaknesses
in
human performance.
This area warrants further management
attention.
In accordance
with 10 CFR 2.790 of the NRC's "Rules of Practice,"
a copy of
this letter and its enclosures will be placed
in the
NRC Public Document
Room.
