REPORT 50-323/94-28
This refers to the inspection
conducted
by H. Tschiltz of this office on
October
19-28,
1994.
The inspection
included
a review of activities
authorized for your Diablo Canyon Nuclear
Power Plant, Unit 2-, facility.
At
the conclusion of the inspection,
the findings were discussed
with those
members of your staff identified in the enclosed
report.
Areas
examined during the inspection
are identified in the report.
This
special
inspection
evaluated
the circumstances
associated
with the loss of
residual
heat
removal flow for approximately
6 minutes
on October
18,
1994.
Within these
areas,
the inspection consisted
of selective
examinations
of
procedures
and representative
records,
interviews with personnel,
and
observation of activities in progress.
The purpose of the inspection
was to
determine
whether activities authorized
by the license
were conducted
safely
and in accordance
with NRC requirements.
Based
on the results of this inspection,
certain licensed activiti,es appeared
to be in violation of NRC requirements,
as specified in the enclosed
Notice of
Violation (Notice).
The violation is of concern
because it involves the
inadvertent
securing of the in-service residual
heat
removal
pump which
resulted
in loss of core cooling during reactor
shutdown,
a serious
safety
concern.
We are concerned
that the cause of the violation involved two
failures
by two members of your operations staff to follow the appropriate
procedure
and that
a contributing cause of the violation was the failure to
conduct
an adequate
pretest briefing.
While we recognize your efforts to
improve procedural
compliance at Diablo Canyon,
we remain concerned
your
efforts
may not
be fully effective.
You are required to respond to this letter
and should follow the instructions
specified
in the enclosed
Notice when preparing your response.
In your
response,
you should
document
the specific actions
taken
and
any additional
actions
you plan to prevent recurrence.
After reviewing your response
to this
Notice, including your proposed corrective actions
and the results of future

the
NRC will determine
whether further
NRC enforcement
action is
necessary
to ensure
compliance with NRC regulatory requirements.
In accordance

Room.
The responses
directed
by this letter
and the enclosed
Notice are not subject
to the clearance
